When enrolling a child in kindergarten or first grade as
a first-time District 123 student, parents must present a certified

for the school district’s record.

Children transferring to District 123 must present
evidence of their grade placement.

school to another within the district, should notify the office
not less than two days before the final day of attendance.
The new home address and the address of the new school
should be supplied.
Attendance Areas
Students must attend the school in the attendance area
in which they reside, unless they are assigned to another
school by the superintendent. Individual school attendance
boundaries are established by the Board of Education on
the recommendation of the superintendent and may be
changed if population conditions warrant it.
Oak Lawn-Hometown District 123 serves those
children from kindergarten through eighth grade who reside
in the area bordered on the east by Pulaski Road, the west
by Central Ave., the north by 87th St., and on the south by
107th St.
Discipline Handbook
The district has prepared a discipline handbook for students
and parents. At the beginning of each school year,
every child will receive a copy of the handbook. Copies are
also available at each school for parents to review.
School Handbooks
Parents are encouraged to refer to their child’s school
handbook for additional policies and practices.
Student Health Records
By law, all children entering school for the first time
and all sixth graders must present evidence of recent physical
and dental examinations and of immunizations against certain
diseases. Such examination is to be conducted within
one year prior to these entry situations.
Students transferring to School District 123 also must
present records, either by transferring them from the former
school or by providing new records.
Physical Examination and Immunization forms are available
for download on the nurse's teacher page of each school website.
Students who do not meet compliance requirements by
the first day of student attendance of each year will be
excluded from school until obligations are satisfied. For
more information about the District’s policy regarding physical
examinations and immunizations, please review Board
policies.
