Every parent/guardian/custodian should register their child/ren at the school they will attend on one of the following dates and times, being mindful that the evening hours are intended for those parents whose work commitments make daytime appointments difficult. |

If you are unable to fulfill the registration commitment on these days, it is imperative to complete this task by August 24, 2007.Please refer to the fee schedule so that you are aware of the various fees that require payment to registration. A spearate check for each child should be made payable to Oak Lawn-Hometown School District 123. |
As the first step of the registration process, every parent/guardian/custodian is required to be present and submit a completed proof of residency form and the following current/most recent documents: |

Required: Photo ID with name and address of parent/guardian/custodian - Driver's license, State ID, or Passport Category 1 (one document required) - real estate tax bill, signed lease, mortgage papers or sales agreement - All documentation must be current/most recent. - AND - Category 2 (three documents showing proper address required) - driver's license, voter registration, library card, home/apt. insurance papers, gas or electric bill, auto registration, credit card/cable bill, or public aid card - All documentation must be current/most recent. |
The principal, staff, and volunteers will be in attendance to help you complete the registration process. |
