OLCHS To Host A Covid-19 Vaccination Clinic On May 13, 2021

Oak Lawn Community High School has partnered with Jewel-Osco Pharmacy to provide a one-day COVID-19 Vaccination Clinic. Vaccinations will be available to the general public for anyone 16 years of age or older. Information on the clinic is below:

  • Facility: Oak Lawn Community High School (Spartan Gym)
  • Address: 9400 Southwest Highway Oak Lawn, IL 60453 (Parking Lot Entrance at 9439 Austin Avenue)
  • Date: Thursday, May 13, 2021
  • Time: 9:00AM – 1:00PM (tentative, will extend if demand warrants)
  • Vaccine: Pfizer
  • Dose Two Date: Thursday, June 3, 2021

Participants 

Pfizer vaccine is for 16 years of age and older only. No one less than 16 years old may register for this event. If a vaccine recipient is under 18 years old, a parent must be present, bring their ID, and sign the consent form for the minor. All populations in Illinois 16 years of age and older can receive the vaccine regardless of insurance coverage or immigration status, including individuals who are undocumented.

What to Bring 

Driver’s license or state issued photo ID, insurance card or Medicaid ID and completed consent form.

Parking and Entrance

Participants should park in the main parking lot along 95th Street and Austin Avenue (Entrance at 9439 Austin Avenue). There will be additional parking in the student parking lot on the west side of Austin Avenue. Participants should enter through the Event Entrance of the building into the Spartan Lobby.

Scheduling

Use link below to schedule your appointment. The time you select on May 13, will be the same appointment time you receive for the 2nd dose on June 3.

Please allow 45 minutes from start to finish for your appointment. Participants will have a 15-minute observation period after receiving the vaccine before they can leave campus.
